PREPARING FOR YOUR APPOINTMENT

Please ensure that you complete all of the required online forms in your client portal at least 48 business hours prior to your scheduled appointment. If these forms are not completed, it is possible we will need to reschedule to a future date. When you arrive for your appointment, please wait for me in the lobby area and text (317)520-1888 that you have arrived.

PAYMENT

Restoration by Grace does not accept insurance. Clients can pay by credit card, check, cash (exact amount only, please), or HSA. Payment will be collected at the end of each session. Paying for sessions in advance is an option if you would like to prepay for you or your child. Session prices are as follows:

• Initial Phone Consult (15 min.): Free

• Intake Session (50 min.): $75

• Individual Sessions (50 min.): $130

• Splankna Sessions (75 min.): $145

CANCELLATION POLICY

Please note that you will be charged a fee for any late cancellations or appointments missed. Please see the Practice Policies form in your client portal for more information.
